Position Summary
The Registered Nurse Care Coordinator serves as the central point of coordination for patient care, working closely with patients, families, providers, and community resources to ensure timely, effective, and comprehensive healthcare services. This role combines clinical expertise, case management, patient education, and interdisciplinary collaboration to deliver exceptional care and improve health outcomes.
The successful candidate will champion patient-centered care while supporting organizational goals, promoting quality improvement, and fostering strong relationships throughout the healthcare continuum.
Key Responsibilities
Patient Care Coordination
- Coordinate and oversee interdisciplinary plans of care for assigned patients.
- Serve as the liaison between patients, families, healthcare providers, and community agencies.
- Promote patient independence through education on medical conditions, medications, treatment plans, and self-care skills.
- Monitor patient progress and ensure care goals are achieved effectively and efficiently.
Case Management & Follow-Up
- Collaborate with providers and community agencies to ensure continuity of care.
- Conduct ongoing care management services and monitor patient service plans.
- Complete annual and semi-annual assessments and provide quarterly care management reports.
- Maintain monthly contact with patients and providers to verify services, evaluate outcomes, and update care plans as needed.
Community Outreach & Health Promotion
- Plan and coordinate specialty clinics, health events, and wellness initiatives.
- Support healthcare promotion activities that improve patient engagement and community well-being.
- Build partnerships with local resources and healthcare organizations to enhance service delivery.
Clinical Support
- Provide relief support within outpatient clinical settings when needed.
- Maintain accurate clinical documentation and patient records.
- Assist with quality improvement efforts and patient care initiatives.
Leadership & Collaboration
- Serve as a resource for colleagues and interdisciplinary teams.
- Demonstrate professionalism, accountability, and teamwork in all interactions.
- Support a culture of compassion, respect, integrity, and continuous learning.

Qualifications
Education
- Associate Degree in Nursing required.
- Bachelor's Degree in Nursing (BSN) preferred.
Experience
- Minimum of 5 years of nursing experience as a Registered Nurse.
- At least 1 year of supervisory or leadership experience.
- 8 years of RN experience preferred.
Licensure & Certifications
- Current Alaska Registered Nurse License.
- Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS) certification required within one year of hire.
Physical Requirements
- Ability to operate standard office and clinical equipment.
- Frequent sitting with occasional standing and walking.
- Ability to bend, reach, twist, stretch, and perform repetitive motions.
- Ability to lift and carry up to 25 pounds independently.
- Normal visual and hearing acuity.
Working Environment
- Primarily office and outpatient clinical settings.
- Occasional travel required throughout the region.
- Travel may include commercial aircraft, small aircraft, snowmobiles, all-terrain vehicles, and boats based on community needs.
